Depending on where you live in US you have to apply to appropriate consulate office through their official visa agency ( not to the consulate directly).

Like lovestosuggest mentioned, looks like the Indian Embassy at Washington D.C has outsourced the visa services to COX & KINGS GLOBAL SERVICES PVT. LTD. (Link)
