For EAD its just previous EAD card copy and Pics if i remember correctly along with forms.
For TPS, along with form, just copy of previous approval of TPS, plus any documents based on your case.

Do you need to provide any additional documents besides copy of Previous EAD, TPS Approval Letter and 2 Passport Photo ?
On I-821 Instructions under general requirements it says
Documentation Exception: If you are filing this application for re-registration (Part 1., Item Number 1.b., of
Form I-821), you do not have to submit any copies of documentation. However, USCIS may ask you for additional
information and/or documentation in certain circumstances.

Please do not forget to submit a copy of your passport as well. copy of your Visa, i-94 and yes copies of TPS approval, EAD card

@Jenny Copy of Passport/Visa is not mandatory for TPS app.
Also, if your HR were okay with accepting you on the first time, they should sensibly accept the Federal Notice to continue your work through Dec. Good luck! (write a polite email and meet them in person)

Has anyone noticed that the form for I-765 has expired? Not sure if there is revised version available or if it’s okay to send the expired one. It says date of expiration is 2/2018. Form I-821 has new date of expiration though.

In different note: passport photos are required right? I remembered sending them last time .

Can we file advance parole doc at a same time when we apply Tps? Or do we need to wait until Tps got approve?

And also I saw filing adress for parole doc is different than Tps filing adress.

Yes you can!! You just gave me a mini heart attack there. I had to go back and read the instructions on the USCIS page again. If you are applying for TPS and AP at the same time, both of those docs can be send to Chicago. If you are applying AP on a pending or approved TPS, then you need to send it to Dallas. It says....

Do you need to provide spouse information on Part 4. Information about your current spouse? It says complete only if you are filing late initial application for TPS. I kinda got confused on that part.

My case #s also start with IOE (i.e. IOE09047*****). I was also used to seeing prefixes such as LIN, WAC, etc.
